The Questionnaires section is where you view and manage the intake form templates assigned to your account. Access via Questionnaires under Tools & Services.
Questionnaires are the intake forms patients complete before a provider reviews their case. Each treatment program has its own questionnaire — an initial intake and usually one or more follow-up forms. Templates are assigned to your account by Wellie MD admin; you can view, publish/unpublish, and in some cases edit them.
Filters and Columns
Filter by All, Published, Draft, and by type (All Types / Initial / Follow-up). Search by name, type, or visit type. Columns: Name, Questionnaire Type (Onboarding or Follow-up), Treatment Type, Visit Type, Slug, Questions, Review (Approved), Status (Published/Draft), Last Updated, Actions.
Publishing
Click Unpublish (red when published) to take a form offline, or publish to make it live. Changes that affect clinical intake questions must be reviewed for compliance before publishing — contact your Account Manager before making significant changes to any onboarding questionnaire.
